Transition to Adulthood





Kent County is rich in resources for adults and children with developmental disabilities.  Identifying those resources can be time consuming and complicated. Transitioning to adulthood can also be complicated.  Parents of children with disabilities spend years relying on the programs offered by the intermediate school system.  The 2004 IDEA Law requires schools to begin transition planning at age 16.  The Individual Transition Plan (ITP) is initiated through an Individualized Education Plan (IEP), starting with the IEP in which a child turns 16 years old.

By developing a transition plan, independent of the school system’s ITP, IS&SS assists parents with the milestone of transitioning to adulthood.  IS&SS can assist in one or more areas of transition, including but not limited to applying for Supplemental Security Income (SSI), identifying potential housing options, and how to initiate guardianship, if appropriate making the transition to adulthood less difficult.
